Based on observation, the licensee did not comply with the section cited above during physical plant tour LPA observed a large knife on kitchen counter drying rack which poses an immediate health, safety or personal rights risk to persons in care.

Based on record review, the licensee did not comply with the section cited above two (2) out of the five (5) residents physicians reports stated they were both bedridden which poses an immediate health, safety or personal rights risk to persons in care.
