What the inspector wrote

Licensing Program Analyst (LPA) Galarza initiated an unannounced Case Management- Deficiencies visit after it was discovered during file review for complaint investigation control # 28-AS-20230630083050 that Dementia residents (R1 & R2) have medical assessments that have not been updated. Resident (R1's) physician report has not been updated since 2020. Resident (R2's) Physician's Report has not been updated since 2021. Per Title 22, Division 6, Chapter 8 Article 12. Dementia 87705 (c)(5) Care of Persons with Dementia . Licensees who accept and retain residents with dementia shall be responsible for ensuring the following: Each resident with dementia shall have an annual medical assessment as specified in Section 87 458, Medical Assessment, and a reappraisal done at least annually, both of which shall include a reassessment of the resident’s dementia care needs. A citation was issued. Exit interview was conducted with Administrator Diana Bautista. A copy of the report and appeal rights were issued.
