What the inspector wrote

Allegation: Licensee listed an incorrect facility license number on the public website. The complaint alleges the facility license number publicized on the website is incorrect. The website was viewed and R1’s records were reviewed. Administration staff and licensee were interviewed. Administration staff and licensee acknowledged the facility license number listed on website https://www.laposadasl.com is not correct. Based on record evaluation, the findings indicate that the facility number listed on the website belongs to a Riverside facility named “Buena Vista Assisted Living” # 331880902, that also has an incorrect facility number listed on the website. The allegation is supported by sufficient evidence. Based on record review and interviews conducted the preponderance of evidence standard has been met, therefore the above allegation is found to be SUBSTANTIATED . Pursuant to Title 22, California Code of Regulations, a deficiency is cited. An exit interview conducted, copy of the report and appeal rights was provided to Executive Director Colleen Rozatti.

In addition, the Resident Handbook lists the previous operator/ licensee's number. This poses a potential, health, safety risks to persons in care.
