What the inspector wrote

Licensing Program Analyst (LPA) Joe Katrdzhyan conducted an unannounced case management visit to this facility. During a complaint investigation visit conducted on 8/31/22, at 10:45am, LPA observed that the auditory chime device located on the front door was inoperable. This presents an immediate health and safety concern for the residents in care as the facility cares for residents with dementia. The following deficiency was observed to be in violation of California code of Regulations, Title 22, Division 6 (refer to 809D) An exit interview was conducted and a copy of this report was provided to the Executive Director along with the Appeals Rights.
