Licensing Program Analyst (LPA) Christian Gutierrez conducted an annual continuation visit. Upon arriving at the facility, LPA met with Caregiver Kevin Brewer who assisted with the visit. On 10/09/2025 LPA conducted an initial visit. During today's visit LPA Gutierrez reviewed the following CARE inspection tool domains: Resident rights/information, resident records/incident reports, and residents with special health needs. Four (4) resident files were reviewed and included physician’s reports and appraisal needs and service plan. LPA reviewed medications for residents, remaining two residents and they are being administered as prescribed by the physician. No deficiency was observed during today’s visit. Exit interview was conducted with Kevin Brewer and a copy of report was provided.
