What the inspector wrote

Licensing Program Analyst (LPA) Blanca Gonzalez conducted an announced Pre-Licensing Inspection visit and met with Applicant Cheryl Carter. This Pre-Licensing Inspection is due to a change of ownership. The facility has an approved fire clearance to be licensed to serve six (6) residents aged 60 and over, five (5) non-ambulatory and one (1) bedridden. LPA and Applicant toured the physical plant interior and exterior. The Pre-Licensing Inspection CARE Tool was used and the following was observed: The facility is a single-story located in a residential area of Glendora. The home consist of living room, dining area, kitchen, three (3) bedrooms, two (2) bathrooms, laundry room, detached garage, front yard and back yard. Laundry room included a locked cabinet for detergent and cleaning supplies. Smoke detectors and carbon monoxide detector observed, tested and are operable. Fire extinguishers last serviced 08/19/2025 continued on LIC809C Appliances such as a microwave, refrigerator and stove were observed to be clean and operating properly. Adequate food supply is stored in the kitchen and consists of the following: 2-day perishables, and 7-day non-perishables. Sharps were observed secured and inaccessible to residents. Centrally stored medications are located in a locked cabinet in the dining area. Dining area was clean with sufficient seating. Living room was observed with sufficient seating for residents and has a fireplace with a covered screen. Grab bars and non-skid floors were observed in the bathrooms. The water temperature was tested in all bathrooms and measured above the required 105-120 degrees F. Licensee adjusted at time of visit. Residents bedrooms have all required furniture, lighting, and bedding. There is closet space for clothing and other belongings. Extra, clean linen were observed in a cabinet in the hallway. Staff and client files are maintained secured. Doors, exits, hallways, and passageways were clear and free of obstruction. The front and back yards were observed to be clean and free of debris. Licensee will provide an outdoor seating area with umbrella for shade and sufficient seating for residents. No pools or bodies of water were observed.. There are no firearms present at the facility. The first-aid kit was observed to be complete. Component III was completed. The Pre-licensing is complete, and the facility has no deficiencies. Exit interview conducted and a copy of this report was provided to Applicant. Accordingly, LPA will submit a copy of this facility evaluation report to the Central Applications Bureau (CAB) for review. If the applicant has questions regarding the status of the application, they have been instructed to communicate with the CAB Analyst assigned to their application.
