What the inspector wrote

Licensing Program Analyst (LPA) Blanca Gonzalez conducted an announced Pre-Licensing Inspection visit and met with Applicant Rathany Suy. This Pre-Licensing Inspection is due to a change of ownership. The facility has an approved fire clearance to be licensed to serve six (6) residents aged 60 and over, five (5) non-ambulatory and one (1) bedridden. Bedroom #2 is the only room that’s designated for bedridden. LPA and Applicant toured the physical plant interior and exterior. The Pre-Licensing Inspection CARE Tool was used and the following was observed: The facility is a single-story located in a residential area of Glendora. The home consists of living room, dining area, kitchen, four (4) bedrooms, two (2) bathrooms, attached garage with laundry area, front yard and back yard. Interconnected, combination smoke/carbon monoxide detector observed located throughout the facility, tested and are operable. Fire extinguishers last serviced 07/07/25 are located in the kitchen and garage. continued on LIC809C Appliances such as a microwave, refrigerator and stove were observed to be clean and operating at the time of visit. Facility was observed to have sufficient supply of two (2) days perishable & seven (7) days non-perishable foods. Sharps were observed secured in a locked toolbox on the kitchen counter and inaccessible to residents. Kitchen area was observed to be clean and free of litter, rodents, vermin and insects. Centrally stored medications are located in a locked cabinet in the dining area. Dining area was clean with sufficient seating. Living room was observed to be clean with sufficient seating for residents. Grab bars and non-skid floors were observed in the bathrooms. The water temperature was tested in all bathrooms and measured 118.9°F and 116.9° F, which is within the required 105°-120° F. All residents’ bedrooms have the required furniture which includes, for each resident, a chair, nightstand, and lights sufficient for reading. Bed linens were clean and in good repair. There is closet space for clothing and other belongings. Extra, clean linen were observed in a cabinet in the hallway. Staff and client files are maintained secured. Resident bed in room #2 was rearranged to not obstruct the exit. Doors, exits, hallways, and passageways were clear and free of obstruction. The front and back yards were observed to be clean and free of debris. Covered outdoor seating area with sufficient seating for residents was observed. There are no pools or bodies of water. There are no firearms present at the facility. The first-aid kit was observed to be complete. Component III was completed. The Pre-licensing is complete, and the facility has no deficiencies. Exit interview conducted and a copy of this report was provided to Applicant. Accordingly, LPA will submit a copy of this facility evaluation report to the Central Applications Bureau (CAB) for review. If the applicant has questions regarding the status of the application, they have been instructed to communicate with the CAB Analyst assigned to their application.
