What the inspector wrote

Licensing Program Analyst (LPA) Sarah Hurt arrived at the facility unannounced on May 7, 2025 at 10:00 a.m. to investigate the above allegations. LPA met with Licensee, Bernardino Luares, and explained the purpose for today’s visit. Regarding the allegation Staff did not prevent resident from eloping. Resident 1 eloped from the facility on 02/03/2025 at approximately 2:35 a.m.. Resident 1's Physicians report documents he is not able to leave the facility unassisted. Based on interviews, observation, and records reviewed the preponderance of evidence standard has been met, therefore the above allegation is found to be SUBSTANTIATED. The following deficiencies are being cited Per Title 22 Regulations. Exit interview conducted Licensee, Bernardino Luares, and a copy of this report along with appeals rights provided.
