On 8/20/2025, Licensing Program Analyst (LPA) M Vega conducted an unannounced inspection at the facility and met with Assistant Administrator - Amber Myers. The purpose of the visit was to deliver findings regarding the above allegations. It was alleged that the Licensee did not ensure facility cleanliness was maintained and Licensee did not ensure facility was free from odors. Based on multiple facility visits and current facility visit facility is clean and free of mal odor. It is determined the allegation is unfounded. This agency has investigated the complaint alleging “Licensee did not ensure facility cleanliness was maintained and Licensee did not ensure facility was free from odors.” We have found that the complaint was unfounded, meaning that the allegation was false, could not have happened or is without a reasonable basis. We have found that the complaint was unfounded, therefore we have dismissed the complaint.
