Routine inspection (multi-day)·February 10, 2026

02/10/2026, Licensing Program Analyst (LPA) Loera arrived unannounced to continue an Annual Required inspection that was initiated on 02/05/2026, and was greeted by administrator, Mayeen Galang. LPA conducted a review of four resident records and P&I monies. All records had the required documentation. LPA conducted review of four staff records/training. Upon a review of records, LPA found all staff to have required initial training as well as current 1st Aid & CPR certification on file. However, facility was unable to show proof of required 20 hours of annual training for staff (S1), staff (S2), and staff (S3). (Deficiency Cited) Facilities last emergency/fire drill was conducted on 01/22/2026. The following deficiencies were observed (see LIC 809D) and cited from the California Code of Regulations and/or Health and Safety Code, Title 22, Division 6 of California Regulation. Failure to correct the deficiency and/or repeat deficiencies within a 12 month period may result in civil penalties. Exit interview conducted and appeal of rights provided. Updated copies of the following documents were requested for facility file and are to be submitted to CCL by 03/10/2026: LIC500 - Updated Personnel Report LIC400 - Affidavit Regarding Client/Resident Cash Resources LIC402 - Surety Bond Updated Liability Insurance Exit interview conducted and copy of report was provided to Administrator.
