What the inspector wrote

At approximately 11:00AM, Licensing Program Analyst (LPA) Chris Arnhold arrived at this facility unannounced to conduct a complaint investigation. LPA met with Business Office Manager Sonia Sandoval. LPA became aware that a new Management company has taken over this facility as of 04/01/2025. Based on a review of records, the Licensee did not notify the Department within the time frame stated in Health and Safety code 1569.15. The new management company, Healthcare Management Partners, HMP, shall work with the Licensee to submit an LIC200 and accompanying documents for the change in management company. Deficiencies are cited from the California Code of Regulations (CCRs), and/or the Health and Safety Code. Failure to correct the cited deficiency(ies), on or before the Plan of Correction (POC) due date, may result in a civil penalty assessment. This report was reviewed with Sonia Sandoval and Appeal rights were given.
