What the inspector wrote

At approximately 1:45PM, Licensing Program Analyst (LPA) Chris Arnhold arrived at this facility unannounced, to conduct a case management visit in regards to an SOC341, Suspected Dependant Adult/Elder abuse form submitted by the facility. LPA met with Executive Director Zenia Shah, interviewed staff and reviewed records. The report states that S1 was given instructions to assist resident, R1, due to the high risk of falls. S1 told two other staff that the resident would be face down on the ground before she would deal with them. R1 did not fall and there were no injuries. S1 was sent home pending an internal investigation. Facility made the proper notifications. LPA requested additional documentation. No citations issued during this visit.
