What the inspector wrote

On this date, Licensing Program Analyst (LPA) D. Ayers contacted the facility to deliver complaint findings via telephone due to COVID-19 and pre-cautionary measures. LPA identified himself and discussed the purpose of the call with Administrator Julia Fonseca. During the course of the investigation, the Department conducted interviews and reviewed records. Staff 1(S1) stated that the requested records were sent the the designated representative on 3/16/2021. The designated representative confirmed that the requested records were received on 3/16/2021. The allegation is Unfounded. No Deficiencies cited. Exit interview conducted, and copy of this report was provided to the Administrator via email.
