What the inspector wrote

On 01/28/2026, Licensing Program Analysts (LPA) Gorban arrived at the facility unannounced to conduct the Required Annual Inspection. LPA was greeted by receptionist, stated the purpose of the visit and met with Administrator (AD). LPA conducted tour of facility inside and out, residents observed in common area after breakfast. The facility was observed to be at a comfortable temperature, clean, in good repair with no passageway obstructions or fire hazards. Fire extinguisher was observed with a service date of: 11/19/2025, disaster drill recorded on December 29th, 2025. Kitchen was toured. An adequate supply of perishable and non-perishable food was observed to be properly stored in walk-in freezer and walk-in refrigerator. Refrigerator temperature was observed maintained at 40 degrees F. and freezer was observed maintained at -1 degrees F. One story building with rooms were toured and observed with adequately furnished with bed, dresser, and adequate lighting. LPA observed securely fastened grab bars next to toilet and all tub/shower areas. Resident’s bathroom temperatures tested at a range of 109 to 112 in bathrooms. LPA completed a tour of medication room. Facility utilizes Alis application to record, track, and monitor medication records. Medications were stored in a locked medication room in a medication cart. MARs and medications were reviewed. A sample of staff files were reviewed to have the required documents. A sample of resident’s files were reviewed to have all the required documents. First Aide kit was observed to have all of the required items. No deficiencies issued during this inspection. Exit Interview conducted. The following documents requested to be updated and submitted to Fresno CCL by 01/30/25: LIC308, LIC500, current Administrator’s certificate. A copy of this report was provided to Administrator, whose signature on this form confirms receipt of these report.
