Licensing Program Analyst (LPAs) Les Xiong and Lady Cabrera conducted an unannounced case management visit. LPAs met with Administrator Tyler Wilds. The purpose of the visit is to address the incident that occurred on 03/21/2021 Unusual incident report was submitted to CCLD by the facility staff to follow-up on the health and safety check of Resident 1 (R1). LPA requested the following documents from facility: 1. Most Physician Report 602 2. Reappraisal/Assessment 3. Admission Agreement
