3. The Department investigated the allegation: Staff coerced resident to take their medication by withholding food. LPA conducted multiple interviews with residents, staff members, and third parties. Interviewees stated they did not witness staff members withholding food to coerce residents to take their medications. 4. The Department investigated the allegation: Staff mismanaged residents’ medication. LPA conducted multiple interviews with residents, staff members, and third parties. Interviewees stated they did not witness staff mismanaging resident's medications. LPA reviewed medication logs and did not find any discrepancies. Although the allegation may have happened or is valid, there is not a preponderance of evidence to prove the alleged violation(s) did or did not occur, therefore the allegations are unsubstantiated. Exit interview was conducted and a copy of this report was provided to Executive Director (ED) Krista Willson and Administrator (AD) Elizabeth Reynaga.
