Incident investigation·August 31, 2022

On 08/31/2022, Licensing Program Analyst (LPA) Walton arrived unannounced to conduct an case management inspection. LPA introduced self, stated the purpose of the visit and requested to meet with the Administrator. LPA met with Administrator, Jessica Johnson. The purpose of today's visit is to follow up on an incident report that was submitted to the Fresno CCL office. LPA reviewed resident and staff records during this visit. LPA is removing the complete file for R1 for the purpose of review and copying and will return the file within 3 business days. No deficiencies issued during today's visit. Exit interview was conducted. A copy of this report was discussed and provided to Administrator, Jessica Johnson, whose signature on this form confirms receipt of this document.
