What the inspector wrote

On 09/26/2022, Licensing Program Analyst contacted Administrator, Jessica Johnson via telephone. LPA introduced self, stated the purpose of the call and requested to speak with the Administrator. LPA spoke with Administrator, Jessica Johnson. On 09/23/2022, due to a technical issue, a citation for background clearance was not issued. The purpose of this report is to issue the citation for the deficiency that was observed during the inspection on 09/23/2022, when LPA observed an uncleared staff in the facility. A deficiency is being a cited in accordance to California Code of Regulations, Title 22, Division 6. A civil penalty was assessed on 09/23/2022 in the amount of $100. Exit interview was conducted. A copy of this report and appeal rights will be provided to Administrator, Jessica Johnson, via email and an electronic read receipt confirms receipt of this document.
