What the inspector wrote

On 01/02/2026 Licensing Program Analyst (LPA) Gorban arrived at the facility unannounced to conduct a Case Management regarding an incident that was reported on 11/05/25. Report stated that multiple residents claimed that during the period of time from August to November of 2025 personal belongings and valuable items went missing from resident rooms. During this visit LPA met with senior executive director Ryan Banner and reviewed the facility records. Based on records review, sixteen residents reported the missing items. Based on records review, prior to the event, eleven out of sixteen residents refused to file personal property inventory list and signed declination form. Additional files requested for review. During this visit LPA was notified that construction is taking place by The Core Group remodeling administration offices, lobby space, clinic, and adding new bistro. Anticipated completion date is August, 2026. Executive director explained the remodeling is not interfering with residents daily living functions. Requested files to be provide by 5pm, 01/09/2029 And exit interview was conducted and a copy of this report was provided to the Executive director.
