What the inspector wrote

On 5/21/2024, Licensing Program Analyst(LPA) D. Ayers arrived unannounced at the facility to conduct a Required Annual Inspection. LPA met with lead Caregiver Maria Carrillo and spoke with Administrator Cyril Tupino via telephone. LPA announced the purpose for the inspection. LPA toured the facility inside and outside. Pathways and doors were clear and free from obstruction. Smoke-detectors and carbon-monoxide detectors were present and operational. Facility was clean and odor free. LPA observed sufficient amount of perishable and non-perishable foodstuffs which were stored properly. Common areas were clean, adequately furnished, and adequately lit. Resident bedrooms were clean and had required minimum furnishings. Resident bathrooms were clean, odor free, and water temperature was within required temperature range. Sharp items and detergents were secured in a locked cabinet in the kitchen. A locked cabinet in the garage was observed to store resident medications, and medications appeared to be administered properly. LPA reviewed facility plan of operations and emergency disaster plan. LPA reviewed resident and staff files. At 10:00 am LPA observed that a bathroom cabinet containing bleach and other cleaning supplied was u nlocked and accessible to residents. It was also observed in the same bathroom that there was no non-skid mat in the shower or available to be placed in the shower. At 11:00am, it was observed that an outdoor area which was accessible to residents contained a standing motorcycle, a broken clothes dryer, and other miscellaneous appliances and items which could be hazardous to residents. During resident file review, it was observed that two out of four residents' files did not contain admission agreements. Also, two out of three residents diagnosed with dementia did not have records of appropriate reappraisals conducted within the last twelve months. See attached for deficiencies cited in accordance with California Code of Regulations Title 22, Division 6. Administrator was provided with a copy of the report and appeal rights.
