What the inspector wrote

On 05/28/2025, Licensing Program Analyst (LPA) V. Gorban conducted an unannounced subsequent complaint investigation to deliver findings. LPA explained the purpose of visit to administrator Andrea Ramirez. During the course of the investigation, LPA conducted a facility conducted a facility tour, interviewed residents, and reviewed records. The Department has investigated the allegations: Staff are not meeting the residents showering needs. Staff are not meeting the residents laundry needs. Based on observations during facility visits and records review, the residents missed their showers due to refusal with additional attempt made later in a day. No missed laundry reported based on records review and residents and staff interviews. Although the allegations may have happened or valid, there are not a preponderance of evidence to prove the alleged violations did or did not occur, therefore the allegations are unsubstantiated. No deficiencies were cited during this visit. Exit interview conducted. Report signed on-site. A copy of this report provided to the facility administrator.
