Licensing Program Analyst (LPA) Erik Gonzalez Campos arrived unannounced on 06/29/2022 to initiate a complaint investigation regarding the above allegations. LPA met with licensee/administrator, Babita Dhawan and Lia Miller. During complaint inspection, LPA toured facility, reviewed records, and conducted interviews. Interviews with staff, residents and hospice nurse did not reveal any evidence of residents being handled in a rough manner while in care, residents not being fed while in care or personal rights violations. Although the allegations may be valid, there is not a preponderance of evidence to prove the alleged violations did, or did not occur. Therefore, the allegation is UNSUBSTANTIATED. Exit interview conducted with Licensee/Administrator, Babita Dhawan and Lia Miller. A copy of this report was printed for the facility.
