Licensing Program Analyst (LPA) Julie Florio conducted a case management- incident inspection on 05/13/25 at approximately 3:00 PM, and met with Mark Sanga, Wellness Coordinator. LPA is conducting a case management visit to obtain more information and conduct a safety and wellness check regarding two incident reports and a SOC341, dated and received by the Department on 05/12/2025, involving Resident 1 (R1) which were self reported by the facility Executive Director. LPA interviewed R1, reviewed R1's records, and obtained copies of documents including a personnel roster/report. No deficiencies were cited during todays visit. Exit interview was conducted with Wellness Coordinator, whose signature on form confirms receipt of document.
