What the inspector wrote

Licensing Program Analyst (LPA) Samer Haddadin conducted an announced visit to the facility to conduct interviews and collect documentation related to complaint number 22-AS-20241209152259. Upon arrival, LPA met with Executive Director (ED) Sara Modugno and explained the purpose of the visit. LPA completed the interviews and obtained the requested documents. An exit interview was conducted, and a copy of this report was provided to the ED .

What does Type A vs Type B mean?

Type A. Serious citation. Imminent or substantial risk to children. The regulator requires corrective action immediately and may impose a civil penalty.

Type B. Lower-severity citation. Corrective action required, no imminent risk. The regulator monitors compliance on the next visit.

    Except as specified in subsection (b), the licensee shall ensure that disinfectants, cleaning solutions.. which could pose a danger to residents are in locked storage and are not left unattended if outside the locked storage. This req is not met as evidenced by: Based on observation, License failed to ensure cleaning supplies were secured. This poses an immediate health and safety risk to residents in care.

    Basic services shall at a minimum include: Personal assistance and care as needed by the resident..., with those activities of daily living such as.. assistance with taking prescribed medications.. This requirement is not met as evidenced by: Based on record review, Licensee failed to ensure residents were provided assistance with taking medication. Residents #1 and 2 missed multiple medications due to refills pending. This poses an immediate health and safety risk to residents in care.
