Eight out of eight witnesses stated that the facility discouraged residents from going out of facility around December 2020 to January 2021, but this did not affect them. Residents were observed to be going in and out of the facility and documented by exit log maintained by the facility. Returning residents were advised to stay inside their room for observation. The Department is unable to ascertain if the allegations occurred as reported. Although the allegation may have happened or is valid, there is not a preponderance of evidence to prove or refute the alleged violation occurred. Thus, the allegation that the facility staff prevent residents from leaving the facility is deemed UNSUBSTANTIATED. LPA Marin conducted an exit interview with AD Goldsmith and copy of this report was left in the facility. (Page 2/2)
