What the inspector wrote

Continuation from Page 1 Deficiency was observed, and citation was issued per Title 22 Division 6 of the California Code of Regulations. LPA Marin conducted an exit interview with AD Goldsmith. LPA discussed the deficiency, citation, and appeal rights. Copies of this report, Deficiency Page and appeal rights were left in the facility. This Department has investigated the complaint alleging facility charged resident a monetary fine not listed in admissions agreement. We have found that the complaint was unfounded, meaning that the allegation was false, could not have happened and/or is without a reasonable basis. We have therefore dismissed the allegation. LPA Marin conducted an exit interview with AD Goldsmith and copy of this report was left in the facility. Page 2/2

This requirement was not met as evidenced by: Based on observation, file review and interviews, the facility missed to provide facility personnel ..to meet resident needs.R1 was observed to be violating the house rules on leaving the facility unassisted; and recent wander episode prior to the incident. Facility missed to provide additional services to address change of behavior of resident. This posed immediate threat on safety of resident in care.
