What the inspector wrote

Licensing Program Analyst (LPA) Claudia Gutierrez made an unannounced Plan of Correction (POC) inspection. LPA was greeted and granted entry by Administrator (AD) Maila Enriquez Soliven and the purpose of the inspection was discussed. LPA is following up regarding deficiencies previously cited on August 6, 2025, during facility's annual/required inspection. Deficiency 87463(a) was cited due to two of three resident files not containing a current Appraisal/Needs and Services Plan. Deficiency 1569.695(c) was cited due to emergency drills not currently being conducted or documented. During today’s visit, AD provided LPA with documentation for fire drill conducted on August 16, 2025. AD also provided LPA with three of three resident files containing current Appraisal/Needs and Services Plan. Two of two deficiencies previously cited are cleared. LPA observed Individual by the name of Jose Ramon Diaz volunteering at the facility. LPA reviewed list of Guardian roster for facility and was unable to locate clearance for Jose Ramon Diaz. Per Guardian, Live Scan application for Ramon Diaz was closed due to incomplete information; a Deficiency was cited on today’s date. Based on today’s observations, one deficiency is being cited per Title 22 Division 6 of the California Code of Regulations. An exit interview was conducted and a copy of this report and appeal rights was left at the facility.
