What the inspector wrote

Licensing Program Analyst (LPA) Jerome Haley made an unannounced visit to complete the required one-year annual inspection. LPA Haley was greeted and granted entry by the sister of the Licensee/Administrator (AD) Liberty Ventura. AD Ventura was unable to attend todays visit. Liberty Home is a one-story residential facility with five bedrooms and two bathrooms. There’s an attached garage and a backyard space. Currently the facility does not have any residents. The last resident left the facility sometime in August 2021. This information was previously reported to the department. During the visit, LPA toured the inside and outside portions of the facility and garage to confirm there are no resident’s being cared for. During the inspection, there were no signs of any resident care being provided. Since the facility has stopped operating, a couple family members of Licensee/Administrator Ventura have moved in. One family member is there to assist the licensee. During the visit, LPA Haley contacted Staff 1 (S1), who confirmed the condition of Licensee/Administrator Ventura, and the current status of the facility. S1 explained, at this time it is unclear if the facility will resume operations or not. However, if the facility continues to operate, S1 understands they need to contact the department prior to accepting a resident. During the conversation with S1, LPA Haley explained to S1 the facility does have fees that are past due and need to be paid. S1 understands and states the fees will be paid. S1 explained to LPA Haley, if the facility does continue to operate, S1 will takeover the role as Administrator and be in charge of the day to day operations. LPA Haley also advised S1 to associate the two family members that are now living in the facility to avoid any confusion during future visits if the facility does decide to continue operations. No deficiencies will be cited as a result of todays visit. An exit interview was conducted with the AD and a copy of the report was provided
