What the inspector wrote

On August 18, 2025, Licensing Program Analyst (LPA) Brandon Lopez made an unannounced Plan of Correction visit to the facility to follow up on a deficiency cited during the annual inspection on July 25, 2025. LPA was greeted and granted entry into the facility by staff after explaining the purpose for the visit. Administrator (AD) Ma Dinah Dela Cruz was notified via telephone and later arrived to assist with the inspection. On today's visit, LPA observed two residents in care and one staff present. LPA observed resident's relaxing in their respective bedrooms. LPA, accompanied by staff, conducting a tour of the interior and exterior portions of the facility. LPA observed the interior to be clear of any obstructions or hazards. LPA also observed the exterior portion to be clear of any obstructions or hazards. LPA observed that the obstructions noted during the annual inspection conducted on July 25, 2025, to have been removed from the property. Based on the observations made during the visit, no deficiencies are being cited per the Title 22 of the California Code of Regulations. Additionally, the deficiency cited during the annual visit conducted on July 25, 2025, was cleared at time of visit. An exit interview was conducted with Administrator Ma Dinah Dela Cruz and a copy of the report was provided.
