What the inspector wrote

Licensing Program Analyst (LPA) Alvaro Ramirez, Jr. made an unannounced visit in conjunction with complaint 22-AS-20230707101059. LPA was greeted and granted entry into the facility and initially met with Caregiver Lucille Nofies. LPA explained the reason for the visit. Administrator (AD) Ma Dinah Dela Cruz arrived shortly after. During today's visit LPA re-issued a citation in conjunction with complaint 22-AS-20230707101059. During the complaint it was alleged that female non-staff is sleeping in common area (living room couch). During the initial 10-Day complaint visit on July 14, 2023, LPA observed that Staff 1 (S1) had a Background Clearance; however, per Licensing Information System (LIS) Facility Personnel Report Summary S1 was not associated to the facility. A $500 Civil Penalty was issued. Based on observations and records reviewed during today’s inspection, one deficiency is being cited per Title 22 Division 6 of the California Code of Regulations. An exit interview was conducted with AD Dela Cruz and a copy of this report along with the Appeal Rights were provided at the time of this visit.
