What the inspector wrote

On today's date Licensing Program Analyst (LPA) William Vanegas made an unannounced visit for the purposes of conducting an annual inspection. Upon arrival LPA was greeted and granted entry to the facility by facility staff. LPA explained the purposes of the visit, and Administrator (AD) Rizalina "Sally" Reyes was notified via telephone and arrived shortly after to assist with the inspection. LPA Vanegas began a tour of the facility and observed the following. AD Rizalina "Sally" Reyes has an active administrator certificate valid from July 01, 2025 through June 30, 2027. This is a one storied home with five bedrooms one of which is a staff room, and four of which are resident rooms, two bathroom, and an attached two car garage. LPA Vanegas observed kitchen area to be clean and free of any mildew and debris. LPA observed there to be a two day supply of perishable food and a seven day supply of non-perishable food on hand a long with a sufficient amount of emergency water on hand, LPA observed there to be a microwave, an electric stove, a refrigerator, dishwasher, dryer and washer. All appeared to be in good condition and tested operational. LPA Vanegas observed all resident bedrooms to be clean and free of any hazards. LPA observed all rooms to have all required furnishings such as a bed, clean linens in good repair; meaning no strains or tares, a chair, reading lamp, and enough storage space to store personal belongings. LPA vanegas observed restrooms to be clean and free of any mildew and debris, LPA observed toilets and water faucets to be operational. LPA observed the hot water temperature to be between 107.8 and 108.9 degrees Fahrenheit. LPA observed bathrooms to have required furnishings such as a shower chair, slip resistant floor mats, and grab bars. CONTINUED ON LIC809C. LPA observed all fire extinguishers to be up to date and fully charged. LPA observed all smoke detectors and carbon monoxide detectors to be in good condition and tested operational. LPA observed first aid kit to have all required items such as adhesive tape, scissors, tweezers, bandages, a thermometer, and a first aid manual. LPA Vanegas toured the outside of the facility and observed there to be a shading seating area available to residents in care upon request. Backyard is large enough to participate in outdoor activities upon resident request. LPA Observed side routes to be free of any debris or hazards, side gates are self latching and unlocked. LPA reviewed three staff files, and five resident files. All files (Staff and Resident) had all required documents and staff had annual required training completed. LPA Vanegas reviewed medication with AD and observed the following: All medications are documented correctly, Per LPA review medications are being administered per physicians orders. LPA observed no discrepancies in regard to medication administration. Based on observations made during today's inspection no deficiencies will be issued per title 22 chapter 8 division 6 of the California Code of Regulations. An exit interview was conducted with AD and a copy of this report was provided to the facility additionally a copy of this report will be mailed to the facility as well.
