What the inspector wrote

Licensing Program Analyst (LPA) Hanna Gough made an unannounced visit to the facility to conduct the required annual inspection. Upon arrival LPA observed the facility to be under construction with a "Do Not Enter" notice on the front door due to it being unsafe to occupy. LPA called Administrator (AD) Florence Tolentino and discussed the purpose of the visit. AD arrived at the facility within 5 minutes of LPAs phone call. AD reported to the Department that a fire had taken place in the garage on May 14, 2025 and all residents were moved out. LPA toured the outside of the facility and was able to look inside windows and the front door and observe that construction was taking place and residents were not residing in the facility. LPA confirmed that care is not being provided at this time. AD informed LPA that the process is going slow due to whenever something is finished they have to wait for the city to come and inspect the finished work before moving on. LPA advised AD to obtain a new fire clearance when the facility construction is completed and to contact LPA after a new fire clearance has been obtained before accepting new residents. No deficiencies were noted pet Title 22 Division 6 of the California Code of Regulations. An exit interview was conducted and a copy of this report was left at the facility.
