What the inspector wrote

Licensing Program Analyst (LPA) Kimberly Lyman conducted an unannounced complaint visit to continue the investigation into the above allegation. LPA was greeted and granted entry into the facility and explained the reason for the visit. During the course of the investigation, LPA toured the facility and interviewed staff and residents as well as reviewed and obtained pertinent documentation such as physician report. Regarding the allegation that facility staff verbally abused resident in care, the investigation revealed the following:Administrator indicated an incident where Resident 1 (R1) was verbally aggressive with Staff 1 (S1) after returning to the facility intoxicated. S1 confirms the incident. S1 denies verbally abusing the resident at any time and two out of two additional staff deny ever witnessing S1 be verbally abusive to residents. Three out of three residents deny any verbal abuse occurring at the facility and expressed satisfaction with all facility staff including S1. Based on interviews conducted, LPA is unable to corroborate the allegation. Therefore, the allegation is deemed unsubstantiated, meaning that although the allegation may have happened or are valid, there is not a preponderance of the evidence to prove that the alleged violation occurred. An exit interview was conducted and a copy of this report was provided to facility.
