Inspector’s narrative

What the inspector wrote

On this date, Licensing Program Analyst (LPA) Samer Haddadin conducted an unannounced annual inspection of the facility. Upon arrival, LPA Haddadin was greeted by Administrator (AD) Arlene Fajardo, who granted entry and was informed of the purpose of the visit. LPA Haddadin toured the interior and exterior of the facility and made the following observations. The facility is a two-story home consisting of six bedrooms, with three designated for staff located upstairs and three designated for residents located downstairs. Additional areas include a kitchen, two bathrooms (one upstairs and one downstairs), a living room with a screened fireplace, a dining room, a family room with a television, and a two-car garage. All resident bedrooms were inspected and found to be appropriately furnished with beds, chairs, and clean linens. Each room provided adequate storage space and was free of tripping hazards. The kitchen was clean, well-maintained, and equipped with operational major appliances. Cleaning supplies, sharps, and knives were stored securely and were inaccessible to residents. The exterior of the facility was observed to be well-maintained. Outdoor furnishings were in good repair, pathways were free of hazards, and the grounds appeared safe and inviting, including a shaded seating area. A gated pool was secured and inaccessible to residents in care. Fire extinguishers were fully charged, with indicators in the green zone, and the most recent service date noted as May 9, 2025. Hot water temperatures in both bathrooms measured between 110.0°F and 116.2°F, within regulatory limits. The facility’s last emergency drill was conducted on June 15, 2025. The food supply was reviewed and found to include at least two days of perishable items and seven days of nonperishable items. A review of client records revealed no discrepancies, and staff files were complete with all required documentation. Based on today’s inspection, no deficiencies are being cited in accordance with Title 22, Division 6 of the California Code of Regulations. An exit interview was conducted, and a copy of this report was provided to the AD .
