What the inspector wrote

On December 5, 2025, Licensing Program Analyst (LPA) Garlli Tat conducted an unannounced visit to the facility for the purpose of a required annual inspection. LPA explained the purpose for the visit and was greeted and granted entry by Administrators Gerald Gustave and Luming Gustave. Gerald Gustave has an effective Administrator's Certificate that expires on August 19, 2026. The facility is a Residential Care facility for the Elderly (RCFE) licensed for six non-ambulatory residents and hospice waiver approved for two. LPA toured the interior and exterior portions of the facility with AD. For this visit, there are no residents in care. There are three shared bedrooms and two full bathrooms. The Administrator and their family is currently occupying the facility. All are cleared and associated with the facility. The annual licensing fee has been paid in full as of December 4, 2025 and has been verified via Licensing Information System. Administrator reported that the facility is currently in non-operation. Administrator would like to keep the home licensed and has continued to pay the annual fees on a yearly basis. The facility has not had residents since 2020. Administrator has agreed that when the facility becomes operational, the Administrator will notify Community Care Licensing Division (CCLD) and ensure the home is ready before admitting residents. Administrator agreed that a case management visit will be conducted for a follow up health and safety inspection before the Administrator accepts their first resident. The facility will remain licensed, but non-operational until then. No deficiencies noted, an exit interview was conducted and a copy of this report was provided and reviewed with Administrator Luming Gustave.
