Inspector’s narrative

What the inspector wrote

Licensing Program Analyst (LPA) Hanna Gough arrived at the facility to conduct the required annual inspection. LPA was greeted and granted entry by staff. LPA met with Administrator (AD) Florence Tolentino and discussed the purpose of the visit. The facility currently has six residents in care. The facility is a one story home with four resident bedrooms, two staff rooms, three bathrooms, living room, kitchen, dining room, backyard with pool and attached two car garage. The facility appears clean, safe and sanitary. LPA observed the kitchen to be clean and free of vermin. LPA observed toxins and chemicals to be locked under the kitchen sink. LPA observed the knives to be locked in a drawer by the stove. LPA observed a two day perishable and seven day nonperishable food supply. LPA observed a fire extinguisher outside of the kitchen charged and with a service date of September 8, 2025. LPA observed all resident rooms had the required components and furnishings. LPA observed a clean supply of linens in the closet by the living room. LPA observed resident bathrooms to have toilet paper, paper towels and non-slip mats in the shower. LPA tested the water to be between 112.4-113 degrees Fahrenheit. LPA observed the centrally stored medication to be locked in the dining room closet and made inaccessible to residents in care. LPA observed the emergency food and water supply in the garage. LPA observed a shaded seating area in the backyard for resident use. LPA observed a pool with a perimeter fence enclosing two sides and is locked on the other two sides with a 5ft wrought iron white fence. LPA observed the backyard to be free of debris and obstructions. LPA observed the sheds in the backyard used for extra storage. LPA and AD tested the fire alarms and carbon monoxide detectors and they were found to be operational. Continue on LIC 809C LPA reviewed staff files and no discrepancies were observed. LPA reviewed resident files and three of six residents need updated needs and services plans. LPA observed the last fire drill was conducted on September 17, 2025. LPA reviewed resident medications and no discrepancies were observed. All staff present are background cleared and associated to the facility. Based on today’s inspection a technical violation is being noted per Title 22 Division 6 of the California Code of Regulations. An exit interview was conducted with AD and a copy of this report along technical violation and LIC858 were left at the facility.
