What the inspector wrote

Based on LPA’s interviews which were conducted, review of documents obtained, and observations, this allegation was UNFOUNDED, meaning that the allegation was false, could not have happened and/or is without a reasonable basis. An exit interview was conducted with S1 Shen. A copy of this report was provided and explained. 2 of the staff interviews also stated that facility will contact 911, but that sometimes a resident will decline in wanting to obtain further medical attention or to get evaluated at the hospital. It was alleged that resident has no bedsheets. LPA conducted a tour of a total of 6 random resident bedrooms and observed that each room had bedsheets. A total of 3 staff interviews were conducted, including the resident's (R1) nurse from an outside agency, of which all 3 interviews stated that resident (R1) was vomiting for a few days, and that R1 removed the bedsheets because there was vomit on it. It was also disclosed that staff re-made R1's bed with R1's bedsheets, but after the bedsheets were cleaned. Based on LPA’s interviews which were conducted, review of documents obtained, and observations, LPA is unable to ascertain if the allegation occurred as reported. Although the allegation may have happened or is valid, there is not a preponderance of evidence to prove or refute the alleged violation occurred; therefore, this allegation is deemed UNSUBSTANTIATED. An exit interview was conducted with S1 Shen. A copy of this report was provided and explained.
