What the inspector wrote

Licensing Program Analyst (LPA) Celine Rodriguez conducted an unannounced case management visit in conjunction to complaints: 22-AS-20230109163057 , 22-AS-20230426155348 and 22-AS-20230711163355. LPA Rodriguez explained the purpose of visit, and met with Chief Operating Officer (COO) Faye Shen. On 7/24/25, LPA Rodriguez conducted a continuation investigation visit for the complaints listed, and during the course of the investigation, LPA Rodriguez observed that 4 out of the 5 resident physician reports reviewed, were incomplete, outdated and not renewed. For today's visit citations were issued per Title 22 Division 6 of the California Code of Regulations. An exit interview was conducted with COO Shen. A copy of this report was explained, and appeal rights were provided during the visit.
