Follow-up on corrections·February 18, 2026

This unannounced Case Management – Deficiencies inspection is being conducted by Licensing Program Analyst (LPA) Sean Haddad for the purpose of issuing citations for deficiencies observed during the investigation into Complaint Control No. 22-AS-20260209140124. LPA met with Administrator (AD) Lauren Chon and Staff #1 (S1) Lupe Jaime and explained the reason for today’s inspection. During the course of the investigation, LPA inspected the facility, interviewed AD, and obtained and reviewed copies of the resident roster, staff roster, and a facility incident report dated January 5, 2026. Per a facility incident report dated January 5, 2026, on January 3, 2026, Resident #1 (R1) was intoxicated, threatened staff and themselves with a knife, and police were called and took R1 to the hospital. However, based on incident reports received at the Orange County Regional Office (OCRO), this incident report was never received at the OCRO and the incident was not reported as required. Based on the observations made during today’s inspection, deficiencies are being cited per Title 22 Division 6 of the California Code of Regulations. See LIC809D. An exit interview was conducted and a copy of this report and appeal rights was discussed with and provided to facility representative.
