What the inspector wrote

Licensing Program Analyst (LPA) Kimberly Lyman conducted an unannounced visit to The Pleasantview Home. The purpose of today’s visit was to conduct the annual required inspection. LPA was allowed entry into the home and explained the reason for the visit. Facility is licensed for 5 non-ambulatory residents and one bedridden. Facility has an approved hospice waiver for 5 residents and the home currently has 5 residents, with 2 residents on hospice. Administrator Mariza Oliva has an administrator certificate expiring on 05/04/2026. LPA Lyman along with Caregiver Sosing toured the facility at 10:02 AM. LPA toured the physical plant, checked food service, facility documentation and the first aid kit. The home consists of 5 resident bedrooms, staff room, living room, dining room, and kitchen as well as 4 restrooms. Resident bedrooms had the required furniture, bed linens and closet/drawer space to accommodate each resident comfortably. At approximately 10:15 AM, LPA observed a bed and drawers/ clothing in an area designated as storage on facility floor plan. Resident bathrooms were checked. LPA observed unsecured Lysol spray in a common restroom. Toilets and water faucets worked properly, grab bars were secure and shower was free of mold/mildew. Water temperature measured between 105 degrees F and 107.4 degrees F in all restrooms. Resident bath towels, toiletries and personal hygiene supplies were adequately stocked. Common areas were clean and clear of hazards. Perishable and non-perishable food supply was checked and adequately stocked at time of visit. Kitchen appliances were operational during today's visit. Fire extinguishers were fully charged. LPA reviewed the infection control and emergency disaster plans and plans are complete and thorough. Facility conducts quarterly emergency drills with the last drill conducted on 09/09/2025. At 10:30 AM, LPA observed unlabeled cleaning supplies in the unlocked laundry room as well as paint in the unlocked garage. Continued on LIC 809-C dated 12/02/2025 LPA observed ample emergency water. Outside grounds were toured. Walkways around the home were clear of hazards. There are no security bars or weapons on the premises. There is ample shaded outdoor seating for residents. Facility does not have a first aid kit. Facility conducts activities in the form of exercise and games. At 1:30 PM, LPA reviewed five resident files and two staff files. All resident files contained required documentation including admission agreements and physician reports. Two out of five residents do not have pre-appraisals in the file and two out of five resident physician reports indicate a risk if those residents are allowed access to toxins. Staff files reviewed contained required documentation including medical assessment/ TB, criminal record clearance and proof of CPR training. LPA reviewed medication storage and administration. Medications are stored in a locked cabinet. LPA observed Resident 2 does not have a corresponding physician order for Senna to show dosing or frequency. Licensee has been asked to provide an updated LIC 500 and liability insurance by 12/16/2025. Based on the observations made during today’s visit, deficiencies are being cited per Title 22 Division 6 of the California Code of Regulations. This report was discussed with the facility representative and a copy was provided as well as appeal rights.

    Based on record review, the licensee did not comply with the section cited above. Facility is administering medication to Resident 2 without an order indicating dosage or frequency which poses a potential health, safety or personal rights risk to persons in care.

    Based on observation, the licensee did not comply with the section cited above. LPA observed cook top does not have protective mechanisms on the knobs which poses a potential health, safety or personal rights risk to persons in care.

    Based on observation, the licensee did not comply with the section cited above. LPA observed storage space on floor plan is being used as a staff room. Fire clearance is based on floor plan. This poses an immediate health, safety or personal rights risk to persons in care.
