What the inspector wrote

On March 5, 2026, Licensing Program Analysts (LPAs) Eboni Bentley and Jessica Cho conducted a case management deficiency visit after observing a deficiency while conducting complaint visit: 22-AS-20260226163914. Per record review, Staff #1 (S1) is not associated to the facility. Per interview with Administrator Rosa Janeth Figueroa stated, S1 began working at the facility as a caregiver on January 8, 2025 as a full-time caregiver and works 5 days per week. During today's visit, LPAs observed S1 working at the facility and providing care to residents in care. A deficiency was cited by Title 22 Division 6 Chapter 8. CIVIL PENALTY ASSESSED. An exit interview was conducted, and a copy of this report, LIC809-D, LIC811, and appeal rights were provided to Administrator Rosa Janeth Figueroa.
