What the inspector wrote

On April 29, 2026, Licensing Program Analysts (LPAs) Eboni Bentley and Jessica Cho conducted a case management deficiency visit after observing a deficiency while conducting complaint visit: 22-AS-20260226163914. Per record review of the Fire Clearance approved on May 21, 2015, the facility made changes to the physical plant without notifying the Department following an annual inspection visit conducted by LPA Bentley on September 2, 2025. Per interview with Administrator Rosa Janeth Figueroa stated, the facility added an office in the garage of the facility, following the 2025 annual inspection and did not notify the Department or provide any documentation. During today's visit, LPAs observed a recently constructed office space with interior walls and a door. A deficiency was cited as per Title 22 Division 6 Chapter 8. CIVIL PENALTY ASSESSED. An exit interview was conducted, and a copy of this report, LIC809-D, LIC421IM, and appeal rights were provided to Administrator Rosa Janeth Figueroa.
