What the inspector wrote

Licensing Program Analyst (LPA) Alvaro Ramirez, Jr made an unannounced case management visit in conjunction with Unusual Incident/Injury Report (UIIR) dated November 5, 2025. LPA was greeted and granted entry by Administrator (AD) Maria P Osuna. LPA explained the reason for the visit. Per UIIR on November 4, 2025, Resident 1 (R1) left the facility and did not return. Per UIIR, on November 4, 2025, at 3:57 p.m. a missing person report was filed with the Garden Grove Police Department. Per UIIR, R1's Conservator and Primary Care Physician were notified. During today's visit LPA reviewed the Physician Report (LIC602) dated October 7, 2025, for R1. Per Physician Report R1 is able to leave the facility unassisted. During the course of the interviews AD stated that R1 had no prior elopement history. Per AD, on November 8, 2025, R1 retuned to the facility. AD reported that on November 8, 2025, R1 was sent out to the Hospital and stated that as today R1 has not been discharge to Westminster Villa. A Health and Safety inspection was conducted, and LPA Ramirez observed no Health and Safety concerns during today's visit. Based on observations made during today’s inspection, no deficiencies are being cited per Title 22 Division 6 of the California Code of Regulations. An exit interview was conducted with AD Osuna and a copy of this report was provided at the time of exit.
