What the inspector wrote

Licensing Program Analyst (LPA) Jessica Cho arrived at the facility unannounced for the purpose of conducting the Required 1-Year annual evaluation using the CARE Inspection Tool. LPA was greeted and granted entry by Caregiver Maria Garcia after stating the reason for the visit. Licensee Renee Radabaugh arrived on premise shortly after. The administrator's certificate for Joshua Radabaugh expires on April 5, 2026. The following was observed during the inspection: This is a single story property with a small loft on the second level located in a residential neighborhood. Facility operates within the conditions and limitations specified on the license. LPA observed two residents in care with no residents receiving hospice service. LPA observed one caregiver on duty and verified fingerprint clearance and association statuses for the caregiver and licensees. All common areas were inspected including the attached two car garage including the loft which is occupied by staff. On the first floor, LPA observed four resident bedrooms and two resident bathrooms. LPA observed the residents' bedrooms were appropriately furnished, beds and bedding supplies were in good condition, adequate lighting was provided, and sufficient storage space for each residents' personal belongings were observed. All bathrooms were found to be in compliance, clean, and operational. The hot water temperature measured at 107.7 and 107.2 degrees Fahrenheit in the resident bathrooms. Toxins, disinfectants, sharps, and medications were secured and inaccessible. LPA observed sufficient two-day supply of perishables and seven-day supply of non-perishable food. LPA toured the exterior portion of the facility. LPA observed the outdoor passageway is free of obstruction. The two exit gates (on the side of the property & driveway) were operational and there were sufficient seating and shading in the patio area. The fire extinguishers were purchased on May 27, 2025. The auditory devices and dual functioning smoke/carbon monoxide detectors were tested and operational on the first floor and loft. LPA observed the emergency food/water and supplies centrally stored in the garage. Emergency drills were conducted quarterly with the the last date on July 11, 2025. The Emergency Disaster Plan (LIC610E) is incomplete. LPA observed the required 'See Something, Say Something' (PUB475) poster posted in the correct size in the entry way. LPA interviewed two out of two residents. Staff interview was not conducted as staff was assisting both residents. LPA reviewed two residents' files and one personnel file in which no discrepancies were found. Medications were audited for the two residents in which no errors were found with the medication administration. However, administrator failed to maintain the doctors' orders for all the routine and PRN medications which were obtained during the visit. Licensees were consulted on the following: to have records readily available for review, to ensure all routine and PRN medication orders are obtained and maintained on file, and to complete the LIC610E. Based on the observations made during today's visit, no deficiency is being cited. Advisory Notes are being issued. LPA also offered additional guidance on questions that were raised by the licensee. An exit interview was conducted with Licensee/Administrator Joshua Radabaugh and Licensee Renee Radabaugh, and a copy of this report was provided at exit.
