What the inspector wrote

On this day, Licensing Program Analyst (LPA) Kevin Saborit-Guasch made an unannounced visit to the facility for the purpose of conducting a case management inspection. LPA met with administrator Michele Goodney and explained the purpose of the visit. A review of the resident records on file within the facility was conducted on a sample of residents R1, R2, R3, R4, R5 and R6. The entire records for resident R6 are absent from the facility after a family member requested the file for the purpose of a separate litigation. The previous administrator provided the family with the original file and has not recovered the file before leaving her position. At this time, the file has not been returned to the facility. The physician report for resident R5 who was recently admitted to the facility is absent from the file also. Based on the review of records conducted at the facility, one deficiency is being cited under Title 22 Division 6 of the California Code of Regulations. An exit interview was conducted and a copy of this report along with appeal rights was provided and left at facility.
