What the inspector wrote

LPA also observe that the dining room was being disinfected. At 9:10 AM LPA observed residents watching television and/or participating in exercises and stretching activities. LPA did not notice malodorous throughout the facility and/or the resident rooms. It was reported via interviews by residents that staff clean daily and/or that staff do their best to clean after the residents. Therefore, based on interviews conducted and documents reviewed the allegations are deemed UNFOUNDED, meaning the allegations are false, could not have happened and/or is without a reasonable basis. LPA Ramirez conducted an exit interview with AD Flores, and a copy of this report was provided to the facility. During the course of the interviews AD stated that staff can start working at the facility when the Live Scan and Background Clearance is in process. During today's visit LPA observed that S1 was working at the facility and S1 identified themselves as the Assistant Administrator. Based on the interviews which were conducted and the records that were reviewed, the preponderance of evidence standard has been met, therefore the following allegation: uncleared staff allowed to work is deemed SUBSTANTIATED. California Code of Regulations, Title 22, Division 6, Chapter 8 is being cited on the attached LIC 9099D. Three $500 Civil Penalties Assessed on LIC421BG. An exit interview was conducted with AD Flores and a copy of this report along with the Appeal Rights were provided at the time of this visit.
