What the inspector wrote

Licensing Program Analysts (LPAs) Hanna Gough and Rose Ruppert made an unannounced visit for the purpose of conducting a health and safety check. LPAs were greeted and granted entry by staff. LPAs met with Administrator (AD) Jenny Truong and discussed the purpose of the visit. During the annual visit LPAs followed up on a death report dated August 22, 2025 for R1. LPAs toured the facility and conducted a health and safety check on all residents in care. LPAs obtained pertinent documentation such as resident POLST, Physicians Report, Appraisal, Assisted Living Waiver (ALW), ALW assessments, Mortuary Information Facts with blank Death Report, and MAR. LPAs filled out the questionable death report and were present at the facility when the fire department arrived after a 911 call for R1 at 0800. An exit interview was conducted with AD Jenny Truong and a copy of this report along with LIC811 was left at the facility.
