On December 5, 2025, Licensing Program Analyst (LPA) Eboni Bentley and Licensing Program Manager (LPM) Lourdes Montoya arrived unannounced for a case management visit to follow up on an incident report received by Orange County Regional Office on November 6, 2025 for Resident 1 (R1). LPA and LPM introduced selves and stated the purpose of the visit to Assistant Executive Director (AED) Monica Aguirre. During today's visit, LPA and LPM were provided full access to R1’s paper and electronic records. A copy of the Coroner's Report and Death Certificate were also requested and the facility stated they would provide copies at a later date. No deficiencies were cited during this visit. An exit interview was conducted with Assistant Executive Director Monica Aguirre, and a copy of this report was provided at exit.
