Regarding the allegation, resident's hygiene needs are not being met, the following has been concluded: The Department conducted seven resident interviews. Six out of the seven residents interviewed denied the allegation and stated that all of their hygiene needs have been met by the facility. However, one of the seven residents interviewed stated that their hygiene needs were not being met due to staffing issues at the facility. The Department conducted seven staff interviews. Six out of the seven staff interviewed denied the allegation and stated hat resident hygiene needs have been met by the facility. However, one out of the seven staff interviewed corroborated the allegation. Regarding the allegation, resident was left out of bed for an extended period of time, the following has been concluded: The Department conducted seven resident interviews. Seven out of seven residents interviewed denied the allegation and stated that they have not heard of any resident being left out of bed for an extended period of time. The Department conducted seven staff interviews. Seven out of the seven staff interviewed denied the allegation and stated that they have not heard of any resident being left out of bed for an extended period of time. Regarding the allegation, facility does not have adequate staffing, the following has been concluded: The Department was unable to obtain any staffing records from the facility for the time period that this complaint was received. The Department conducted seven resident interviews. Six out of the seven residents interviewed denied the allegation and stated that they believe there has been adequate staffing at the facility. However, one of the seven residents interviewed corroborated the allegation and stated that there were previous issues with staffing at the facility. Regarding the allegation that, facility does not have PPE, the following has been concluded: The Department conducted seven resident interviews. Seven out of the seven residents interviewed denied the allegation and stated there has been sufficient supply of PPE at the facility. The Department conducted seven staff interviews. Seven out of the seven staff interviewed also denied the allegation and confirmed there has always been a sufficient supply of PPE at the facility. Based on the evidence gathered during the investigation, the Department is unable to ascertain if the allegations occurred as reported. Although the allegations may have happened or is valid, there is not a preponderance of evidence to prove or refute the alleged violation occurred; therefore, the five allegations are deemed UNSUBSTANTIATED. An exit interview was conducted with Executive Director Melanie Washington and a copy of the report was provided. Based on the evidence gathered during this investigation, the complaint is UNFOUNDED, meaning that the allegation was false, could not have happened and/or is without reasonable basis.
