CONTINUED...The physician report for R1 dated 10/7/2022 page 4, section 14. Mental condition indicates "Yes" under confused/disoriented with mild cognitive impairment. Therefore, based on the preponderance of evidence gathered through interviews, documentation review and observations conducted by LPA Quiroz, the allegations that the “Client was being emotionally abused while in care”is deemed UNSUBSTANTIATED, meaning that although the allegation may have happened or is valid, there is not a preponderance of the evidence to prove that the alleged violation occurred. This agency has investigated this complaint. No deficiencies cited during today's visit. An exit interview was conducted with Caregiver Rhodora Cabrera and with (L/AD) Uldarico Almiranez via telephone, and a copy of report and LIC 811-Confidential Names were provided at exit.
